Consiliar Palliative Team

The consiliary palliative team is available to hospitalized patients at the Hospital Jihlava. The team offers help and support in caring for people with advanced terminal illness.
The aim is to alleviate the difficulties, trying to address both the physical manifestations of the disease and everything that is related to the severe illness. Care is focused on the quality of life of the patient and his/her family.

The team offers support in the form of a consultation, a supportive conversation or a meeting with loved ones. The palliative care team consists of doctors, general nurses, a social worker, a clinical psychologist and a chaplain. They can discuss their situation with the patient and their family so that they understand it, and plan the next steps after discharge from hospital during the hospital stay. They help with the follow-up services that are needed so that a person with an advanced illness can live with dignity and in an environment where they are most comfortable.
